For the Rapids City Times

WISCONSIN RAPIDS – The Wood County Health Department seeks input from county residents.

The department is required to complete a Community Health Assessment every five years and local hospitals and clinics – Marshfield Health Systems and Aspirus – are required to do this every three years

From the results of this survey a Community Health Improvement Plan is created. The last version of the plan can be found here.

The deadline to take this survey is May 19, and those participating should live in Wood County.

The survey helps inform our community teams about the current state of health in Wood County communities.

The survey takes less than eight minutes to complete and may be found here https://www.surveymonkey.com/r/WoodCountyCHA2019.
